Chest Pain History & STEMI Criteria Reference Card – SOCRATES Mnemonic + ECG Lead Area Guide
Confidently assess chest pain and identify STEMI with this double-sided clinical reference card, designed for paramedics, nurses, student clinicians, and emergency staff. One side focuses on structured history taking using the SOCRATES mnemonic, while the reverse outlines UK/EU STEMI criteria and ECG lead-to-heart-area correlations.
Side One: SOCRATES – Chest Pain History Mnemonic
Each letter in the SOCRATES mnemonic is paired with tailored question prompts, guiding clinicians to perform a focused and thorough assessment of chest pain:
S – Site: "Where exactly is the pain?"
O – Onset: "When did it start? Sudden or gradual?"
C – Character: "How would you describe the pain? (Sharp, dull, crushing?)"
R – Radiation: "Does the pain move anywhere else?"
A – Associated Symptoms: "Any nausea, sweating, dizziness, breathlessness?"
T – Timing: "How long has it lasted? Has it changed?"
E – Exacerbating/Relieving Factors: "What makes it better or worse?"
S – Severity: "On a scale of 1–10, how bad is the pain?"
Perfect for OSCEs, patient reports, or streamlining handovers in the field.
Side Two: STEMI Criteria & ECG Lead Area Reference
A quick-access guide to recognising STEMI based on UK/EU guidelines, including:
ST Elevation Criteria:
• ≥1 mm in two contiguous leads and/or ≥1.5 mm for woman, ≥ 2mm men >40yrs & 2.5mm men <40yrs in V2–V3
• New or presumed new LBBB with symptoms
• Reciprocal changes supporting diagnosis
ECG Leads & Corresponding Heart Areas:
• II, III, aVF = Inferior
• V1–V2 = Septal
• V3–V4 = Anterior
• V5–V6 = Lateral
• I, aVL = High lateral
• Posterior = Look for ST depression in V1–V3 or use posterior leads
Designed to support STEMI recognition and ensure timely reperfusion decisions.
