• Criteria: ≥1 mm in ≥2 contiguous leads and/or ≥1.5 mm for woman, ≥ 2mm men >40yrs & 2.5mm men <40yrs in V2–V3
• Description: Sign of transmural infarction; hallmark of STEMI
ST Depression
• Criteria: ≥0.5 mm depression in ≥2 contiguous leads
• Description: Suggests ischaemia or reciprocal change
T-Wave Inversion
• Criteria: Inverted T waves in contiguous leads
• Description: May indicate ischaemia, infarction, or reperfusion
De Winter T-Waves
• Criteria: Up-sloping ST depression with peaked T waves in precordials
• Description: Anterior STEMI equivalent; LAD occlusion
Wellens Syndrome
• Criteria: Deeply inverted or biphasic T waves in V2–V3, no ST elevation
• Description: Indicates critical LAD stenosis; high-risk
Left Bundle Branch Block (LBBB)
• Criteria: Wide QRS >120 ms, dominant S in V1, broad R in I/V6
• Description: Can mask MI; use Sgarbossa criteria
Right Bundle Branch Block (RBBB)
• Criteria: rSR’ in V1, wide QRS, slurred S in I/V6
• Description: May coexist with MI, especially in anterior infarction
